Medical Summary Sex: Female Scan: Negative Estimated age: 8 as per previous owner Fleas: No flea dirt. Hx: Mammary masses, bloody foul smelling discharge from vulva and from mammary glands S: Active, friendly, lays on the floor for belly rubs, allows all handling O: BAR H, BCS 8/9, MMs pink and moist, CRT <2 sec EENT: No discharge OU, AU, nose. Very mild tartar and gingivitis. PLNs: Popliteal LNs diffusely enlarged H/L: NSR, NMA. Panting, quiet lung sounds. Occasional snort. Abd: Soft, full, no pain on palpation. M/S/I: Amb x4. 2 cm dia round lesion on the left flank a ring of very thin fur and then a central region of thicker fur. No erythema, crusting or discharge. UG: Female, well developed mammary glands and vulva. Thin red discharge from vulva. Cloudy tan to red discharge from all mammary glands when expressed. There is a soft oblong mass associated with the left 4th mammary gland, about 3 5 cm long. There is a firm, multilobulated mass associated with the right 4th mammary gland, about 5-8 cm dia. Wood?s lamp no fluorescence A: 1. Mammary masses, mammary discharge, enlarged popliteal LNs R/O mammary neoplasia vs. mastitis (less likely) 2. Vulvar discharge R/O metastasis of neoplasia vs. in heat vs. open pyometra 3. Skin lesion on left flank R/O ringworm vs. other Short-term prognosis: Poor to grave, depending on final diagnosis for mammary and vulvar conditions P: 1. Consider placement vs. EHR. If treatment is sought, pt will need CBC/chemistry, FNA of mammary masses, possible abd rads 2. DTM culture 3. Pyrantel 6 ml 4. Clavamox 375 mg PO BID x7 days 5. Baytril 272 mg PO SID x7 days 1088
