s/o: dog is high heartworm positive. He has healing abrasions on the rear upper thigh/hip area. Both eyes has mucoid discharge and the eyelids appear to be rolling inward
a: high heartworm positive and healing wounds and conjunctivitis OD possible secondary to entropion
p: will treat with Erythromycin Ophthalmic ointment OU BID x 10 days
