Evaluation Notes: Dog displays sociability by coming over for treats and affection with a loose body and a wagging tail. She is easy to leash and walk outside where she walks well on leash and is seemingly house trained. 04/30/2025 Dog is housed in an office where she displays sociability with all handlers by coming over for treats and affection with a loose body and a wagging docked tail. She willingly prompts handlers for affection and will climb into handlers lap for continued affection with a loose body and a wagging docked tail. She is low energy by her interest in sleeping in bed majority of day with brief breaks for potty walks, to prompt affection from handlers and for meal time. She is crate trained, house trained and will bark as a signal that she needs to go outside for potty. She is easy to leash and walk outside where she walks well on leash and pees frequently due to medical. She tires quickly when on walks possibly due to medical and/or heat outside. When outside she is reported to be fearful of planes flying over head. Dog has been observed displaying dog reactivity while on walks by growling, barking and lunging toward passing dogs with a tense, frontal body and while making direct eye contact. She is easy to redirect and can be walked away where she is hyper vigilant and looks back over shoulder frequently. Behavior possibly due to medical and/or hyper-vigilance. 05/14/2025 Dog displays sociability by coming over for treats and affection with a loose body and a wagging docked tail. She is easy to leash and walks slowly on leash where I am able to verbal lure her to continue walking. She meets a smaller dog friendly male dog at barrier where she displays reactivity by puffing cheeks and posturing with a tense, frontal and aligned body, alert ears and while making direct eye contact. She is easy to redirect and is walked away from barrier with verbal encouragement. She look over shoulder frequently as she is walking away and is easy to return to kennel. Behavior possibly due to barrier reactivity and/or general dog reactivity. 05/14/2025 Vet Exam Notes: Pt on vet exam list for large lumps on the neck & chest, possible broken tail, and possible prolapse BAR, friendly, pendulous golf ball-sized mass on right thoracic mammary, semisoft palpable mass on dorsal aspect of anus, tail in fixed position over rump, ambulatory r/o Cyst, chronic injury, neoplasia, other Advise mass removal, continue monitoring 04/29/2025 Pt presented for discharge coming from the anal region BAR, moderate mucopurulent vulvar discharge, anal sacs full, per Foster defecation WNL but will urinate small amounts several times r/o UTI, anal sacculitis, vaginitis, other Anal glands expressed yielding thick brownish-yellow exudate, beginning course of antibiotics 05/06/2025 Pt presented for OHE, currently undergoing tx for susp anal sacculitis and susp UTI Surgery declined - Requested rescheduling in approx 1-2 weeks on a 1/2 dkt combo day, mass removal advised Continue meds until completed 05/07/2025 Pt on vet exam list for hacking cough and eye discharge, currently undergoing treatment for susp anal sacculitis and susp UTI BAR, coughing and sneezing during exam, moderate mucoid nasal discharge, mild epiphora OU, ambulatory r/o CIRD, infectious tracheobronchitis, other Adding course of doxycycline
