Evaluation Notes: 09/06/2026 - Dog is nervous upon approach barking with a tense body and trembling. As I open the door to
her kennel, she slowly comes over and is able to be leashed. She walks outside slowly as she is nervous.
She greets a male dog she was impounded with with a loose body and wagging tail.
Once in a play yard, she adjusts displaying sociability by coming over to all handlers with a loose body and
wagging tail. She jumps up in handlers' laps for affection and attempts to lick faces.
She is returned to kennel.
Vet Exam Notes: 09/21/2026 - Pt on vet exam list for coughing, nasal discharge, distended belly, skin irritation, and not eating
QAR, calm, coughing during exam, nares clear, multiple scabbed wounds and scars on dorsum and thoracic
limbs, abdomen appears WNL, ambulatory, reported to have eaten chicken
Susp chronic injuries
Recently began treatment for susp CIRD, continue monitoring eating habits
