Shelter STAFF OBSERVATIONS 02/13/26 Dog participated in playgroup. Dog is playful. 02/15/26 12:50 - Dog walked by volunteer. Notes: "Very friendly, likes to play with grass and loves treats." 03/10/26 Dog participated in playgroup today. 03/19/26 Dog participated in playgroup style today. 04/02/26 Dog participated in playgroup style today. 04/11/26 Dog participated in playgroup style today. Shelter MEDICAL NOTES 01/14/26 15:15 ER - Scanned no chip. 02/09/26 13:45 📍 used catchpole for administering boosters + vecra. bar. 02/21/26 11:25 Pt was written on ACT board due to nasal discharge, Starting on Doxy 300mg PO SID x 10 days, Per protocol. 02/26/26 11:56 following up on white board - blood in diarrhea reported by ACT-in kennel with 2 other Dogs. + Giardia Snap Test (+)+Parvo Snap Test (-) Per Protocol RVT +Drontal : 1 tab. SID x 3 days 04/03/26 14:54 patient appears to show limping and slight weight bearing on rt front paw, does have few bite wounds, ertheyma and possible swelling near elbow region near radius and humearus. does have laceration on ad, did appear to have hemoarraging in inner AD. asked DR.for a check in on patients lacerations and bite wounds. patient was sedated with ttdex 0.75cc/ml IM, telazol bottle # 26-168. patient was clip and cleaned, rt front leg was flushed with sterile water. AD was cleaned and checked for any injurr. per dr., okay to provide caprofen 2.2cc/ml sq, convenia 2.47 and rx rimadyl (caprofen) 50mg bid po for 5 days. patient was provided reversal 0.38cc/ml IM 04/09/26 10:59 Pt was brought in for sedated ear check. Telazol 0.75ml (26-184) Butorphanol 0.39ml (26-05) IM. Reversal .39ml IM. AU ear cleaning, AS fluid inside, removed fluid, instilled Osurnia in AS per DVM. Ear drums intact AU. 4DX Snap - Negative. Wounds around legs and in ears seem to be healing nicely. Gave booster vaccines, and flea tx. Right inguinal cryptorchid. 04/09/26 11:16 Gave a second reversal /39ml IM per Pt was not awake after giving reversal 20 minute ago.
